Mama Tokens — Programmable Incentives for Maternal Health Programmes
Mama Tokens is a protocol, not a consumer app. Clinics use it to drive verified ANC attendance and report outcomes. Funders use it to disburse capital only when milestones are confirmed — with every transaction on-chain. Startups use its wallet, analytics, and field infrastructure as shared rails for their own health products. Country deployments (MAMAZWE in Zimbabwe, MAMARSA in South Africa) bring the full stack to specific populations and partners.

Why Incentivise Maternal Health Behaviours?
In low- and middle-income settings, structural barriers — distance, cost, and social norms — prevent pregnant women from completing antenatal care even when services exist. Incentive-based ANC programmes consistently increase uptake, improve screening adherence, and reduce maternal and neonatal mortality in studies across sub-Saharan Africa, South Asia, and Latin America.
Generic cash transfers improve access but create limited accountability for funders and no signal about which health behaviours actually changed. Mama Tokens replaces the cash envelope with programmable, behaviour-linked tokens: disbursement only happens when a specific milestone is verified, and spending is restricted to approved maternal health services.
Behaviour-linked disbursement
Tokens are issued only on verified milestone completion — not on enrolment or time elapsed alone. Funding follows action.
Programmable spend rules
Redemption is restricted to approved service categories, preventing leakage into non-health expenditure. Rules are set per programme.
Funder-configurable
Issuance logic, spending windows, and eligible services are configured by funders per deployment — no code changes required.
On-chain auditability
Every issuance and redemption is recorded on the Stellar ledger. Funders, auditors, and programme staff see the same immutable record.
How the Protocol Works
Mama Tokens is a rules engine for programmable health financing — not an end-user app. Here is how a programme is designed, deployed, and reported on.
Programme Design & Eligibility
Funders or programme leads configure the protocol: which beneficiary populations are eligible, which clinical milestones trigger token issuance (ANC visits, HIV screenings, adherence check-ins, module completions), the token value per milestone, approved spending categories, and redemption windows. Mama Tokens is the rules engine — not a consumer app.
Verified Issuance on Stellar
When a CHW or clinic staff member confirms a milestone event, the configured rules fire and MAMA tokens are issued to the beneficiary's wallet on the Stellar network. Issuance is immediate, tamper-evident, and requires no manual reconciliation. Every event is recorded on-chain from the moment it happens.
Provider Network & Outcome Reporting
Tokens are redeemable only at vetted providers within the programme's network — mobile clinics, hospitals, pharmacies — for defined maternal health services. Every redemption event is logged on-chain. Programme managers and funders access real-time dashboards on milestone completion rates, spend categories, and outcomes without waiting for manual reports.
Country Programmes
Each country programme runs the same core Mama Tokens protocol with a localised brand, a dedicated partner network, and a selection of add-on components (wallet, analytics, LMS, AI triage, field infrastructure) chosen for that country's context.
Mama Tokens Zimbabwe
ZimbosAbantu Healthcare on Wheels
A full-stack country programme built in partnership with ZimbosAbantu mobile clinics — the deepest current end-to-end deployment of the Mama Tokens protocol. Adds AmmaWallet, Medical Aid Analytics, AI Symptom Checker, SproutX LMS, and Secure Field Infrastructure on top of the core incentive layer.
- Maternal care, HIV management & STI treatment
- Rural and peri-urban mobile clinic network
- CHW-led milestone verification
- Full add-on stack: wallet, analytics, LMS, AI, infra
Mama Tokens South Africa
DPI Health Hackathon 2026 · NHI Readiness
A country programme designed for South Africa's NHI implementation context, developed under the DPI Health Hackathon 2026 Health Financing & Access lane. Adapts the core protocol for NHI-aligned digital payments, referral tracking, and equitable access — with stack components tuned for South Africa's mobile payment infrastructure.
- NHI-aligned incentive and payment design
- Referral tracking as tokenised events
- Digital payments for maternal stipends
- Interoperable with national digital health systems
Future Country Programmes
Open to partners
The Mama Tokens protocol is designed for replication. Each country programme uses a localised brand (MAMA + country code), selects the add-on stack components relevant to its partner network, and configures milestone criteria and redemption categories to match local clinical guidelines and funder requirements.
- Replicable protocol, configurable per country
- Localised brand per deployment
- Modular add-on components selected per context
- Funder-configurable issuance and spend rules
The Mama Tokens Stack
The core protocol plus five add-on components. Country programmes select the components relevant to their partner network and clinical context. Zimbabwe (MAMAZWE) is the first full end-to-end deployment of all six layers.

Who We Work With
Mama Tokens serves three distinct partners simultaneously. Each gains something the others cannot easily provide alone.
Clinics & Hospitals
Increase uptake. Reduce no-shows. Access traceable programme funding.
- Increase completed ANC visits per patient cohort through verified incentive design
- Receive per-visit funding from programme donors without manual claim submission
- Access structured outcome data on your patient population without extra reporting burden
- Integrate with mobile clinic, fixed facility, or hybrid CHW delivery models
Funders & Impact Investors
Disburse only on outcomes. Audit every transaction. Report with confidence.
- Funds release only when a verified milestone is confirmed — zero leakage to unverified events
- Immutable on-chain record from disbursement to service delivery event, auditable by any party
- Real-time outcome dashboards aligned to SDG 3, PEPFAR, and impact finance reporting standards
- Co-design milestone criteria and eligible service categories to match your funding mandate
